Introduction: Why Age-Based Women’s Health Screening Matters
Women’s health needs change significantly with age. A screening that’s essential in your 40s may not be necessary in your 20s—and vice versa. Understanding women’s health screening recommendations by age helps you stay proactive about your well-being and catch potential issues early.
At Genesis IVF Penang, part of the Alpha IVF Group, we believe reproductive health is an essential part of overall wellness. This guide outlines the key screenings Malaysian women should consider at each life stage.
Your 20s: Building Your Health Baseline
Your 20s are about establishing healthy habits and getting baseline measurements.
Cervical Cancer Screening
Start at age 21. Women should begin cervical cancer screening at 21 and continue every 3 years with a Pap smear, or every 5 years with an HPV test for those aged 30 and above. Sexually active women should be screened regardless of marital status.
Blood Pressure
Check at least every 2 years. If your reading is 120-139/80-89 mmHg, annual checks are recommended. Higher readings require more frequent monitoring.
STI Screening
Sexually active women should test for chlamydia, gonorrhea, and HIV based on lifestyle and risk factors.
Breast Self-Awareness
Perform monthly breast self-examinations. Learn what’s normal for you so you can notice changes.
Dental and Eye Checks
Visit the dentist every 6-12 months. Eye exams every 2 years if you have vision issues.
Your 30s: The Decade of Proactive Monitoring
Your 30s bring new screening priorities, especially if you’re planning a family.
Cervical Cancer Screening
Continue Pap smear every 3 years or HPV test every 5 years (ages 30-65). The HPV DNA test is now available free through LPPKN’s Klinik Nur Sejahtera for women aged 30-65, using a convenient self-sampling method.
Clinical Breast Examination
From age 30 onward, clinical breast exams (CBE) are offered opportunistically at government health facilities. If you have risk factors, examinations should be every 6-12 months; otherwise every 2 years.
Fertility Baseline (If Planning Pregnancy)
If you’re considering pregnancy in your 30s or want to understand your fertility status, ask your doctor about AMH testing, antral follicle count, and hormone panel. These are not routine for all women but valuable for family planning.
Blood Sugar and Cholesterol
If you have risk factors like BMI >23 (for Asians), high blood pressure, or family history of diabetes, screening is recommended.
Thyroid Function
Thyroid issues become more common in your 30s and can affect energy, weight, and fertility. Your doctor may include thyroid testing in routine blood work.
Your 40s: When Screening Intensifies
Your 40s mark a shift toward age-related screening priorities.
Mammogram
Women aged 40-49: Mammogram can be done upon request, especially for high-risk women. Women 50 and above: Routine mammogram every 2 years is recommended. The LPPKN Mammogram Subsidy Programme offers subsidized or free mammograms for women aged 40-70, with priority for high-risk women. Household income below RM10,000/month qualifies for full subsidy.
Cervical Cancer Screening
Continue Pap smear every 3 years or HPV test every 5 years until age 65.
Osteoporosis Screening
The Malaysian Clinical Practice Guidelines 2022 recommend bone density screening (DEXA scan) for women aged 50 and above. If you’re in your 40s with risk factors like early menopause, low body weight, or family history, discuss early screening with your doctor.
Cholesterol Screening
Recommended starting at age 45 for women without risk factors, or age 20 if risk factors are present. Normal results should be repeated every 5 years.
Eye Health
If you have diabetes, annual eye exams are essential. Otherwise, every 2 years or as advised.
